Final Examination Informational Packet
Please Read and Complete The Following Information
First and foremost: Complete the IASHEP Course Sign-In Sheet Located On Page
Nine (9) of this final examination packet and turn it into the proctor of the test.
Dear IASHEP Professional Member or Student:
You are now eligible to take the exam for the International Association of Safety,
Health & Environmental Professionals (IASHEP) professional certification for
which you applied. Please read these instructions carefully, as they will guide you
through the process of scheduling your exam and provide you with important
information about the IASHEP professional testing policies and procedures. Please
note: scheduling, rescheduling and cancellation instructions and policies may vary
by proctoring test administration type (Paper Based Testing vs. Computer Based
Testing).
If you have taken an IASHEP professional course through an IASHEP Authorized
Learning Center, you will take the proctored, timed and open book test at the end
of the course. If you qualified to take an IASHEP professional course through
IASHEP Distant Learning process, then you are responsible to find a Test Proctor
who will agree to proctor your final professional examination. Once you have
obtained a proctor and had the proctor complete the IASHEP proctor agreement
forms and return them back to IASHEP. Once IASHEP administrative staff receives

the proctor agreement document back and signed, IASHEP will contact the
proctor and send them the course final examination. The proctor will then set a
time for which you will take the IASHEP professional examination.
Grounds for Test Site Dismissal
The test center administrator/supervisor or proctor is authorized to dismiss you
from an examination administration (test site). If this was to happen, your exam
scores will be voided or IASHEP may take other appropriate action. You may be
dismissed or subject to disciplinary action if there is a reasonable basis to
conclude that you have engaged in any of the following conduct:
1. Using or attempting to use someone else to take the test.
2. Failing to provide acceptable personal identification.
3. Having access to, or using, notes or any prohibited aid related to the test.
4. Creating a disturbance (disruptive behavior in any form will not be tolerated;
the test administrator/supervisor has sole discretion in determining whether
specific conduct constitutes disruptive behavior).
5. Communicating, in any manner, with another person other than the test
administrator/supervisor or proctor about the test during the administration,
including attempting to give or receive assistance.
6. Attempting to remove scrap paper from the testing room or tearing the scrap
paper if provided.
7. Eating or drinking in the testing room.
8. Leaving the testing room or test center vicinity without permission.
9. Removing or attempting to remove, examination related material, or portions

of a test in any format from the testing room.
10. Attempting to tamper with a computer if one is supplied by the testing center.
11. Engaging in any dishonest or unethical conduct, such as cheating.
12. Failing to follow any other examination administration regulations set forth in
IASHEP certification department policies given by the test administrator /
supervisor or specified in any examination materials.
The International Association of Safety, Health & Environmental Professionals
reserves the right to take all action including, but not limited to, barring you from
future testing and/or canceling your scores for failure to comply with the test
administrator/supervisor’s directions. If your scores are canceled, you will be
notified of such action and its basis, and your examination fees will not be
refunded.
Examination Security & Confidentiality Policy
The examination, answer sheets, worksheets, and/or any other test or testrelated materials remain the sole and exclusive property of the International
Association of Safety, Health & Environmental Professionals (IASHEP). These
materials are confidential and are not available for review by any person or
agency for any reason. When you submit an application, you agree to abide by
the International Association of Safety, Health & Environmental Professionals
(IASHEP) Examination Security & Confidentiality Policy. The examination, answer
sheets, worksheets, and/or any other test or test-related materials remain the
sole and exclusive property of the International Association of Safety, Health &
Environmental Professionals (IASHEP). These materials are confidential and are
not available for review by any person or agency for any reason. When you
submit an application, you agree to abide by the International Association of
Safety, Health & Environmental Professionals (IASHEP) Certification
Application/Renewal Agreement (found in your respective International
Association of Safety, Health & Environmental Professionals (IASHEP) certification

or recertification handbook).
The Certification Application/Renewal Agreement states: “Furthermore, I agree
not to discuss, debrief or disclose, in any manner, the specific content of the
International Association of Safety, Health & Environmental Professionals
(IASHEP) examination questions and answers, to any individual.” If you are found
to have violated the examination confidentiality policy or the Certification
Application/Renewal Agreement, the status of your credential could be affected.
Disciplinary actions against violations include revocation of your credential or
permanent suspension from any International Association of Safety, Health &
Environmental Professionals (IASHEP) credential examinations. If you have any

Report Irregularities or Wrong-Doings
Although International Association of Safety, Health & Environmental
Professionals (IASHEP) tests are administered under strict supervision and
security measures, examination irregularities may sometimes occur. You are
required to contact us as soon as possible to report any observed behavior that
may lead to an invalid score-for example, someone copying from another test
taker, taking a test for someone else, having access to test questions before the
exam, or using notes or unauthorized aids. All information will be held in
confidence.
Outside / Independent Test Center Instructions Check-in procedure:
From time to time, the International Association of Safety, Health &
Environmental Professionals (IASHEP) may use outside or contracted independent
testing centers to proctor our examinations. If the International Association of
Safety, Health & Environmental Professionals (IASHEP) or one of the IASHEP
Authorized Learning Centers agree to proctor an examination the following is

required before a test is issued.
On the day of the examination, please arrive one half hour before your scheduled
appointment. You must sign in, present the required identification. You may also
be asked to provide the examination confirmation notice received when
scheduling the appointment.
Important Information about Your Identification: In order to be admitted into
the testing center, you must bring a valid, non-expired government-issued
identification. Your identification needs to include:
• English characters/translation
• your photograph and
• your signature
If your government-issued identification does not display a photograph or a
signature, an additional, secondary identification may be used, which includes a
photograph and/or signature (whichever is missing from the government-issued
identification), and your name printed on the identification. All identification must
be current (e.g., not expired).
All forms of identification being presented at the testing center must match your
name exactly as it appears on the scheduling notification. Your identification
documents must be in good condition, and cannot be bent, frayed, taped, cracked
or otherwise damaged in any way. You will not be permitted to test if the name
on your identification documents does not exactly match the name on your
scheduling notification, or if your identification is damaged. Neither International
Association of Safety, Health & Environmental Professionals (IASHEP) nor its
IASHEP Authorized Learning Partners or IASHEP independent contracting test
centers will make any exceptions to this policy. If you do not provide the
appropriate and/or matching identification, you will not be permitted to test. If
you still wish to take the exam, you will be required to apply for reexamination
and pay the reexamination fee in order to take the exam at a later date, when

valid identification is available.
Testing Instructions
The IASHEP professional member / student has a total of four (4) hours once the
IASHEP professional member / student starts to take the examination to finish
it. The IASHEP examination may be an open book test depending on the
certification course but the professional IASHEP member / student may not use
any outside notes once he / she starts the final examination unless specifically
granted permission from International Association of Safety, Health &
Environmental Professionals (IASHEP).
Breaks: The IASHEP professional member / student member may be allowed to
leave the testing center or room after two hours to take a ten (10) minute break
or in case of an emergency, the IASHEP professional member / student must turn
in the course materials / test to the proctor before leaving the examination room.
If an IASHEP professional member or student must leave the room, the test
proctor must pick up all of the testing materials until he / she returns. The IASHEP
professional member / student text book must not have any highlights or marks in
the text book when taking one of IASHEP professional level test. If the IASHEP
professional member / student text book or materials is marked or highlighted,
do not allow the IASHEP professional member / student to take the final
examination, instead, please contact IASHEP at either (612) 801-1032 or (520)
568-5565 and IASHEP will send the proctor a new unmarked book for this
important higher-level examination.
Retesting: If you fail in your first attempt of the IASHEP professional certification
examination, don’t panic. You still have two more attempts left to retake the
IASHEP professional written test within one year of your eligibility period.
Please note that you have to use these two re-attempts within one year of
your eligibility period.

Failing to do so will ban you from filling out the IASHEP exam application for one
year. Even if you don’t attempt the exam or you failed in the remaining two
attempts, you cannot sit the test for the next year.
Retesting fee structure: The second or third-time re-examination fee for any
IASHEP professional credential will be as follows:
• For an IASHEP Member Fes is $275 USD
• For a Non-Member of IASHEP fee is $375 USD
Assessment Center Testing Accommodations
Comfort Aids. You may bring certain personal items defined as “minor comfort
aids” to the assessment center to enhance your comfort during the testing
session; however, you must present the items to testing staff for inspection prior
to testing. Bringing and using such items in the assessment center are not
considered testing accommodations.
No advance notice is required, and you do not need to submit a Testing
Accommodations Request Form or supporting documentation. Items considered
“comfort aids” include:
• Cough drops/glucose tablets (You must unwrap all cough drops before
bringing them into the testing room.);
• Eyeglasses;
• Hearing aids;
• Insulin pump attached to your body;
• Pillow or cushion;
• Tissues. You must bring your own comfort aids; these items will not be
provided at the assessment center.
Physical Disability.
If you require accommodations other than those listed as “comfort aids” or
“minor alterations” and you are requesting the accommodations due to a physical
disability (e.g., visual impairment), you must:

1. submit a completed Testing Accommodations Request Form, identifying the
disability and the specific accommodations requested; and
2. submit a signed statement by a licensed professional, whose license or
credentials are appropriate to diagnose the disability (e.g., ophthalmologist for
visual impairment).
This statement, which must be written on the qualified professional’s official
letterhead, must indicate the following:
a. the disability for which testing accommodations are being requested
b. the recommended test administration and/or portfolio modifications that are
specifically related to the disability and are reasonable in the context of this
certification test
